Edu Psychology assessment report for ESF application

Anyone with the experience being asked to submit the edu-psych assessment report for application to proceed at ESF primary
Applied for Year2 and after meeting with the principal and vice principal, they said my child may require more support hence will need the report in order to apply for special needs supports from the ESF center.
Anyone with the similar experience? How would you do?
